Tax Forms
As tax season begins, we will be offering on-demand tax forms. Please use the form below to request both tax forms and instruction booklets. Once your request has been received, we will notify you via phone or email, and you can pick the forms up at the Circulation Desk. Please note that both the federal and state instruction booklets, once they arrive, come in limited quantities, so we will only be able to offer these for free while supplies last. They will be added to this form when they are in-stock. If there are none in, they will not be available to order on this form until there are more available. Once the total supply has run out, instruction booklets will need to be printed in-house and will be subject to our regular printing fees.
Need to look at the forms before you make a request? Click HERE to visit official IRS website and look at PDF copies of the federal forms, schedules, and instructions (with tax table). Click HERE to visit the official Massachusetts government website and view the Massachusetts tax booklet information (with personal income Form-1/HC forms).
Please note, the Library receives a limited quantity of both state and federal booklets and they will be available on a first-come, first-served basis. Free 2022 booklets (both state and federal) are no longer available.
Tax Form Request Form
Even as the library reopens, we will still be offering to print patron documents and various tax forms. Print requests will be available for pick up at our Holds Station once the patron is notified.
